The following is a list of cases compiled with limited resources and restricted access to records, it is therefore by no means complete. We believe there to be many more care deaths/serious injuries around the country.

It is interesting to note that Salford Social Services were also prosecuted by the HSE in 1991 for failing to report an 'accident'. As they were also prosecuted for failing to report Malcolm's death this raises the question of just how many of these 'accidents' go unreported.
